Principal Investigator submits ancillary study proposal Data Only Application electronically to the FHS Research and Publications Committee coordinator.

The investigator of proposals that are approved by the Research and Publication Committee but involve no participant burden (though they may use scans or repository data), and minimal clinical implications will be notified directly by the coordinator after consultation with the chair of the Research and Publication Committee chairman.

Prior to receipt of data and/or biosamples (but not required at the time of application), the requester must also complete and submit a Data and Materials Distribution Agreement. This agreement must be completed before any data or lab samples will be released. An original hard copy of the signed Data and Materials Distribution Agreement should be mailed to:
