Appropriate applications and documents are provided for ancillary study proposals. The investigator should review the section on Ancillary Studies Overview and determine which application is appropriate for the proposal. All forms, including supplementary and supportive materials, should be completed according to the instructions. The proposal applications often require materials beyond completion of a proposal form. These additions can include, for example, for DNA applications, IRB approval, and distribution agreements signed by the investigator's institution. All required materials should be completed and submitted electronically to the FHS contact designated on the application form.
Application Forms
The PI should decide if the proposed study is best described as one involving the use of DNA, subject burden, outside funding and/or biosamples or if it fits into the category as small and requiring no or minimal funding. The PI also should decide if laboratory specimens are needed. The application forms for the studies and laboratory specimens are given below.
