Dementia was defined using DSM-IV criteria but we additionally required that 
severity be at least 'moderate' by Framingham criteria (>=1 on the Clinical 
Dementia Rating Scale) and that symptoms be present for >= 6 months. 
Alzhiemer's Disease (AD) was defined using NINCDS-ADRDA criteria.
