Steps to Enroll in the LEGACY program

1. Review the FHS LEGACY brochure and this website to learn more about what enrollment in the program entails.
   
   
2. If you are interested in participating in the program, an important part of the enrollment process is to identify the next-of-kin who will be your partner for this program. When you have identified your next-of-kin, we can provide LEGACY materials to them at your request so that they can review the program details to best support you in your decision.
   
   
3. Any questions by yourself or your next-of-kin can be answered by our LEGACY team – please contact us at FHSheart@bu.edu or 800-536-4143 and we can either talk to you on the phone or set up a virtual meeting with a member of our team and your next-of-kin to help answer your questions.
   
   
4. If you decide to enroll in the study, we will set up either a virtual or in-person meeting with a FHS LEGACY coordinator, you, and your assigned next-of-kin according to your preference to obtain your consent. We will email you a link with our e-consent form to be signed by the end of the call if meeting virtually.
   
   
5. Once you provide consent, you and your next-of-kin will receive a copy of your signed consent form and additional details. We will keep in contact with you and your next-of-kin annually with program check-ins and any updates.
