The Framingham Heart Study designs data collection forms for each examination cycle for each cohort. Information is recorded on the appropriate form during the course of the examination for every participant in attendance. Framingham recently transitioned from a paper data collection to REDCap, a direct data entry system.

Framingham recently transitioned from a paper data collection to REDCap, a direct data entry system. When REDCap exam forms were used for data collection, the exam forms are presented in two formats: REDCap layout and annotated form layout.
